Windows binary rewriting fixes, Windows process detach, neither works

- Windows binary rewriting works from the Dyninst side, but the runtime
library can't resolve trap-based instrumentation yet, we need to tell
the runtime-library how to find the table of trap mappings.
- Windows process detach doesn't work, I'm not sure why.
